PURPOSE
The aim of this course is to enable a learner to produce an occupational health and safety incident investigation based on organisational requirements, an incident and a report based on that incident.
COURSE OBJECTIVES
At the successful completion of this course, the learner will be able to:
- Explain why workplace accidents/incidents need to be reported and investigated.
- Explain the concepts of multiple causation and root-cause analysis.
- Detail an incident investigation process using a hypothetical incident.
- Produce an incident investigation report based on a documented incident.

Health and Safety Representative – Stage Three can also be completed online.
NOTE: This course provides credits towards the NZ Certificate in Workplace Health & Safety Practice (Level 3).
